Contract


This is the contract you enter into when agreeing to work with me.

My website contains details of my insurance and GDPR policy. If you can not access the internet, I can provide this in printed form.

Therapy is a regular session of 50 minutes currently provided online/phone. Please ensure you have a quiet and confidential place for therapy.

Therapy is a confidential space; however, personal, legal and practical limits of confidentiality will be discussed in our first intake session and covered here. Confidentiality is superseded by court orders, acts of terrorism, drug trafficking and money laundering. Confidentiality is also superseded if I believe you are at risk or harming yourself and or others.

I keep sparse notes electronically on an encrypted laptop. Occasionally some information may be shared with referring agencies/organisations, for case studies and for the purposes of learning and development. At times I may use anonymised client information for examples, case studies, and educational purposes. If you do not wish for your data to be used in this way you have the option to opt-out. I have a separate form for audio and video recordings, and I will discuss this with you if required. You may also request notes or data to be sent to a third party.

I will only contact you to discuss appointments, to settle complaints, in case of emergency and to chase unpaid fees. Please state a contact preference (Email, text, Telegram or WhatsApp) and the privacy status of your preferred method. If you wish to contact me, please email, Telegram, WhatsApp or text. I will reply as soon as I can regarding rebooking or administrative issues.

I may refer you to an alternative provider if I feel it would be more beneficial for you (for example if I feel I am not experienced or qualified regarding your presenting issue). I may, on occasion, need to seek guidance from a fellow professional.

Sessions require payment in advance and have a 48hr cancellation policy. Due to Covid-19 all sessions are conducted online by FaceTime or Zoom. Fees can be paid by cash, BACS or card payment. Please note that for data security, we don’t hold records of your banking or card details; these are covered by the respective companies GDPR policies (card payments are handled by iZettle).

Online appointments mean you calling me, or joining the provided zoom link without reminders from myself. If you don’t make contact within 20 minutes of the start time, the session will not be undertaken and is still chargeable. Connectivity or technology issues are still chargeable if they are at your end. Any form of intoxication means the therapy is cancelled. Sessions outside of this period may be rearranged by negotiation and at my discretion.

Fees are £60 a session for individuals and £80 for couples. Payment details will be provided on confirmation of booking.

Please ensure payment is made 48hrs before your session.

Should something happen to me, I have a therapeutic executor who will contact you. They will have access to each client’s encrypted contact details should the need arise.

Prior to your first session, I require the following information via email to booking@dcwellbeing.co.uk;

  • Name
  • DOB
  • Emergency Contact
  • Doctors details
  • Details of any prescribed drugs

Providing these details is confirmation of your agreement to our sessions and is electronic acceptance of this contract.
